Abstract: The article presents a case study of a 44-year-old man with a history of type 2 diabetes, presented to the hospital emergency department with complaints of malaise, fatigue, heartburn, and decreased exercise capacity. He had been taking the dipeptidyl peptidase 4 inhibitor sitagliptin 100 mg, and metformin 1,000 mg twice daily for the past 2 years. On physical exam, he was found to be uncomfortable and in mild distress, and he exhibited tenderness to palpation over the epigastric area.
